Samantha Ammons is a board-certified Family Nurse Practitioner, medical aesthetics provider, hormone specialist, and passionate entrepreneur. As the founder of Revive, Aesthetics and Wellness she has grown the practice into an expanding brand with four locations, increasing access to high-quality aesthetic and wellness care for a broader community.
With over 10 years of experience in the medical field including emergency medicine, family practice, aesthetics, and wellness, Samantha brings a comprehensive, well-rounded approach to patient care. She has thoughtfully curated an exceptional team with a shared commitment to safe, patient-centered treatment and clinical excellence.
Samantha remains actively involved in her practice, continuing to see patients across multiple Revive locations, while consistently expanding the brand with innovative services and offerings. Outside of her professional work, she is deeply dedicated to her family and community and is always on the lookout for the best ways to optimize airline and hotel points.
Debra Alfaro

Originally from New Jersey, Debra began her career in dermatology and has spent over 15 years immersed in all things aesthetics. With a deep understanding of skin health and facial anatomy, Debra specializes in injectables, advanced laser treatments, and full-facial rejuvenation, treating all skin types with a personalized, results-driven approach.
In addition to her clinical work, Debra is a certified Cartessa trainer and is passionate about educating both patients and providers. She believes that knowledge, trust, and subtle artistry are the foundation of beautiful, natural results.
Outside of the treatment room, Debra enjoys spending time with her husband and five children, girls’ nights, traveling and trying new restaurants.

My name is Tamara, and I’ve been a nurse since 2010. I have worked as an LPN in a variety of specialties, gaining diverse experience that has shaped my passion for patient care and wellness.
Having traveled to multiple countries and met people from many different backgrounds, I’ve developed a deep appreciation for personalized, holistic care. When Sammi invited me to join Revive and help bring expanded services and experiences to our community, I knew it was an opportunity I couldn’t pass up.
I am the proud mother of three beautiful children and am blessed with a loving, supportive husband. At Revive, my focus includes IV hydration, VI Peel treatments, and SkinPen microneedling. I am passionate about helping clients look and feel their best through customized, compassionate care.

Sam Ferree is a wife, mom of two, and Family Nurse Practitioner with a passion for aesthetics and patient connection. She’s been in the aesthetics field for nearly five years, after working as a nurse in both the Pediatric ICU and Labor & Delivery. Sam is known for her warm, down-to-earth approach and her ability to make patients feel instantly comfortable. When she’s not in the clinic, you can find her chasing live music at a great concert, sipping an (overpriced but totally worth it) coffee, or throwing on a ball cap and soaking up time with her family. As a mom of two amazing kids, my approach to wellness is deeply rooted in balance, longevity, and self-care—shaped by my love for yoga and working out. I’m committed to delivering subtle, natural results that honor individuality, because true beauty is confidence supported by whole-body wellness.
